How to Use Magill Forceps

In cases of severe airway obstruction, prompt action can make the difference between life and death. While basic life support (BLS) procedures often suffice for the removal of superficial foreign bodies, deeper obstructions require more specialised tools. The Magill Intubation Angled Forceps, paired with direct laryngoscopy, is a trusted method for such challenges.

Steps for Using the Magill Forceps:

  1. Prepare the Patient and Equipment: Ensure the patient is in the 'sniffing' position.
  2. Hold the Laryngoscope: Grip the laryngoscope handle with your left hand.
  3. Insert the Laryngoscope Blade: Gently open the patient's mouth and introduce the blade. When advancing, exert gentle upward traction along the handle's axis at a 45° angle. Avoid using teeth or gums as leverage points.
  4. Visualise the Foreign Body: Watch the laryngoscope's tip until the obstruction becomes visible. Do not surpass the vocal cords. Use suction to clear any secretions and improve your view if needed.
  5. Introduce the Magill Forceps: With the tips closed, insert the forceps into the mouth.
  6. Retrieve the Foreign Body: Identify and grasp the object while maintaining a direct visual on it.
  7. Extraction: Securely remove the obstruction using the Magill forceps.
  8. Ensure a Clear Airway: Inspect the airway to confirm no residual debris remains. Subsequently, remove the laryngoscope blade.
  9. Ventilate the Patient: Make attempts to ventilate the patient following the procedure.

Remember, in situations where basic life support methods don't prove effective, or the obstruction remains embedded deeper within the airway, the Magill forceps combined with laryngoscopy often stands as the best chance for a successful resolution. Always approach the procedure with care and precision.
